At MKH, we don't use templated demands because we believe they don't do our clients justice. You deserve for your attorney to take the time to approach your demands in a detailed manner. For a little context, the “demand package” in a personal injury case is the document that usually prompts settlement. In it, the attorney tells the story of the client's case, including their pain and suffering. Many large personal injury firms have a template they use and then they simply add on the medical records without really telling what the client went through.
